What kind of care does the center offer?

The Jane Pauley Community Health Center provides a wide range of primary health care services—including treatment of common illnesses and minor injuries—as well as behavioral care services.

Here are just some examples of the services available (this is not a complete list):

  • Checkups and wellness screenings—Annual physicals, well-child checks, cholesterol and diabetes screenings, blood pressure, laboratory tests
  • Vaccinations—Flu, pneumonia, hepatitis B, tetanus, childhood immunizations
  • Treatment of common family illnesses—Colds, viruses, ear infections, sore/strep throat, skin rashes, seasonal allergies, bladder infections, upper respiratory infections, sinus infections, insect bites, acne
  • Chronic disease management—High blood pressure, diabetes, emphysema
  • Behavioral care services—Screening for anxiety, depression, substance use and other chronic and acute issues; in-house treatment and referrals as necessary

Who provides the care?

Patients at the Jane Pauley Community Health Center may be seen by a family physician, nurse practitioner, physician assistant, registered nurse or a psychologist.

Community Health Network provides our staff. Laboratory and hospital services are available through the Community system.

What does it cost?

The Jane Pauley Community Health Center’s services are intended to be affordable and available to all, regardless of income or insurance coverage. Most major insurance, Medicaid and Medicare are accepted. Financial assistance programs are available for uninsured patients, based on income and family size.

Do I need an appointment?

The center strives to accommodate patients as quickly as possible when they need care. However, it is best to call for an appointment whenever possible, so that the staff may plan for your arrival and maximize your convenience.
