Insurance Cards
Please bring your insurance card(s) with you to each appointment. This includes Medicare, Medicaid and private insurers.
Payment/Financial Assistance
Required co-payments will be collected at the time of your appointment. Financial Assistance forms are available for those without insurance and unable to pay the full amount. Patients applying for the Financial Assistance Program may be asked to fill out a Personal Financial Statement.
